RESILIENCE CARE ("we", "our", "us") offers a text messaging service to send you medical questionnaires, appointment reminders, information on your healthcare facility or our activities, and service updates. Messages sent from this program are informational and transactional in nature and are not marketing messages.
By providing your mobile phone number and opting in to the service, you give Prior Express Consent to receive recurring text messages from RESILIENCE CARE to the phone number provided, including messages sent using an automatic telephone dialing system (ATDS).
- You understand that consent is not a condition of purchasing any goods or services.
- The texts you receive will be directly related to your care, appointments, or services with RESILIENCE CARE or your healthcare facility.
Message frequency will vary depending on your specific interactions with your healthcare facility, such as programming of your medical questionnaires, upcoming appointments and follow-up care, as well as updates and information RESILIENCE CARE or your healthcare facility would like to share with you.
Message and data rates may apply for any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, contact your wireless provider.
You can cancel the SMS service at any time. Just reply STOP to any message you receive. After you send the message "STOP" to us, we will send you one final text message to confirm that you have been unsubscribed. After this, you will no longer receive text messages from this program. If you wish to rejoin, please sign up again as you did the first time.
For assistance or more information, reply HELP to any message you receive, or contact us directly at support@resilience.care. For medical help, you need to contact directly and immediately your healthcare facility or 911. Our team will not be able to assist you in this matter.
Your mobile information, including your phone number and information about your text messages, will be treated in accordance with our Privacy Policy.
We will not share or sell your mobile information with third parties/affiliates for marketing or promotional purposes. Your health information is protected by the Health Insurance Portability and Accountability Act (HIPAA).
Carriers, such as AT&T, T-Mobile, Verizon, and others, are not liable for delayed or undelivered messages.
